UHC announces reductions across listed plan categories. Its MA list includes 92507, 92508 and 92526; the separate D-SNP list includes 92507 and 92508. Plan exceptions require portal verification.
Practice or reimbursement impact: Do not infer all SLP approvals ended or that removal for 92507 transfers to its 2027 replacement codes.
