Latest focused CMS review and news update: (Central)
Latest payer source-access pass: November 4, 2026 · 7:53 PM Central. 30 endpoints attempted: 26 retrieved, 4 failed. 4 Aetna policy/date sections reviewed; no prior-version comparison or new payer adoption established. Source status and access gaps →
Earlier CMS review: Verified the CMS final-rule release record and the official final-rule pediatric telehealth passage. Pediatric timing, age, coding choice and valuation narrative reviewed on PDF pp. 228–234; final dollar rates, unit implementation and payer adoption remain incomplete. Separate bounded payer pass: 30/30 entries attempted, 28 retrieved and 2 failed (Molina marketplace: 404; Health Alliance: 502); 27 index/landing attempts and 3 partial document retrievals, with 0 full prior-version comparisons. No new payer-policy change established; not every source was reviewed today. Scheduled attempts are not guaranteed completed reviews.

October 28 focused scan
Focused source scan · October 28, 2026

Compared with the October 27 briefing, this focused check did not establish a new material therapy policy change. Previously published notices remain below with their original dates. CMS’s regulation index still lists the 2027 physician fee schedule proposal; a final rule or final payment values were not verified in that index.

Checked: CMS PFS index · ASHA reimbursement news · BCBSIL provider news · HFS public notices · IDEC EI · EI training program · Provider Connections · ISBE special education · ISBE licensure · Aetna newsletter · CountyCare notices · Molina Illinois authorization page · UHC commercial index · Meridian policy index.

Coverage limits: index and landing-page checks are not full policy comparisons. The IDEC news URL was unavailable; the EI Central Billing Office and Cigna were not directly reviewed. Dates on individual records remain their own last successful source checks.

What Today’s Check covers

One running briefing for important policy and payer developments, Illinois practice updates, advocacy opportunities, deadlines and education. Broader monitoring scope was added October 28, 2026; this is not a claim that every source has been checked.

Items link to official sources and retain their own dates and verification limits. Proposals are not final policy; older guidance newly found is labeled separately. Check the event organizer before registering. Open the events calendar →

Selected-source refresh · October 30, 2026

New: ASHA audit-preparation resource and BCBSIL Medicare Advantage authorization announcement. RP040 old/new maintenance language is now compared, but SLP applicability remains unresolved. December 8 ASHA education was added to the calendar. CMS’s index continued to list the 2027 proposal; final payment values were not verified.

Sources checked and coverage limits

Direct checks included ASHA announcements/toolkit, CMS index and proposal listing, BCBSIL notices/current and pending RP040, HFS selected notices, Provider Connections/EI rate notice, EITP announcements, CountyCare’s addendum notice and selected Aetna Better Health Illinois notices. These are selected-source checks, not every payer or full policy comparisons.

EITP retrieval recovered. EI CBO exposed only an informational landing page; program notices remain unreviewed. Molina’s current memo contents and HFS’s notice table were not exposed. An HFS stuttering URL returned conflicting content, so its older record was not re-dated. Illinois DOI’s bulletin page did not expose current table rows. eviCore’s therapy index was accessible and lists SLP guidelines effective November 5, but their text and payer adoption were not compared. Cigna and UHC were not rechecked in this catch-up. CountyCare’s revised January 1 addendum is portal-only; its public notice does not establish changed therapy services. No new enacted legislative change was established in this bounded check.

2027 Medicare rule watch

Checked November 4, 2026: OIRA lists the final rule as pending review, received September 29. The CMS index lists the proposed rule; a 2027 final rule was not listed in the sources checked. Final payment values are not confirmed.

OIRA review status, Federal Register public inspection, formal publication and effective dates are different stages. AMA RUC recommendations are not final Medicare payment determinations; commercial and Medicaid payer adoption must be checked separately.

November 4, 2026CPT 2027: five treatment-code pairs and the pediatric optionReference updated/checked: November 4 · Code-set effective: January 1, 2027 · ASHA publication date not stated
ResourceNational

CPT 92507 remains in effect through December 31, 2026; the replacement family starts January 1, 2027. Each pair has an initial 30-minute base and additional 15-minute add-on: fluency 92654/+92655; speech sounds 92656/+92657; language 92658/+92659; combined speech and language 92660/+92661; voice/upper airway/resonance 92662/+92663. These are short service labels, not official CPT descriptors.

92508 remains the group-treatment code, with revised stand-alone wording. Do not use the new codes early. Confirm payer timing, authorization conversion, benefits and contracted rates.

Separate Medicare option: G0903 is untimed with no minimum duration for eligible patients before age 22. CMS permits either G0903 or the new CPT pathway; the pathways are mutually exclusive. Final CMS clarifications and valuation details.

November 4, 2026CMS releases 2027 final rule: pediatric code G0903Public display: November 4 · Federal Register publication scheduled: November 9 · Effective: January 1, 2027 · Checked: November 4, 2026 (Central)
Policy updateNational · MedicareFinal rule

CMS has released CMS-1848-F. The final rule identifies G0903 for individual pediatric speech-language pathology treatment and adds it to the Medicare Telehealth Services List for 2027. This updates the earlier GSLPP proposal watch. The pediatric response clarifies eligibility through age 21 years and 364 days, before the 22nd birthday (PDF p. 229).

Why it matters: The final federal rule is available now. Commercial and Medicaid adoption, authorization, payment and contractual rates require separate payer evidence. Telehealth listing alone does not establish every billing requirement.

Next step: Review the official rule and addenda; confirm each payer’s implementation. CMS clarifies that G0903 is untimed, with no minimum session length or midpoint threshold; the 60-minute valuation is not a required treatment duration. CMS permits selection of G0903 or the new timed CPT family for eligible pediatric services; the pathways are mutually exclusive (pp. 229–230). CMS retains work RVU 1.30 and PE RVU 0.97 (pp. 233–234); these are not dollar payment amounts. G0903 is an always-therapy service subject to therapy MPPR (p. 231). Final dollar rates, unit implementation and payer adoption remain under review.

Premera pauses planned therapy payment reduction
Posted Nov 2 · Dec 4 implementation paused · Checked Nov 3
Policy updateWashington / Alaska

Status: Public payer guidance; individual claims/payment not confirmed.

Premera says clinician feedback prompted a pause of its Always Therapy Multiple Procedure Reduction policy. It commits to at least 90 days’ notice before any eventual implementation. This is a regional comparison, not an Illinois rule; code-level SLP payment impact is not established by this notice.

Why it matters: Provider feedback can influence implementation.

Next step: Watch for a revised decision and verify the affected product and codes.

Nov 1, 2026UHC Medicare Advantage therapy policy: code-date conflict needs clarificationApproved Oct 14 · Effective and index-published Nov 1 · Source checked Nov 4

UHC's November Medicare Advantage bulletin classifies MMP079.20 as an “Updated” policy, a category it says does not change coverage guidelines. The policy's November 1 revision history says it added a notation marking 92507, 92508 and 92526 “deleted 09/30/2026.” ASHA says 92507 remains in the CPT set through December 31 and 92508 continues in 2027. The notation's meaning, when providers were notified, and claims handling remain unresolved; the prior UHC version was not available for a full comparison.

Check the member's Medicare Advantage plan, applicable delegated review and current written payer guidance before relying on any code or authorization assumption. Policy approval is not proof of payment or 2027 payer adoption.

ResourceNational

ASHA highlights a Medicaid audit-preparation toolkit addressing documentation, medical necessity, goals, school-based billing and audit red flags. This is professional preparation guidance, not a new coverage, authorization or reimbursement requirement.

Why it matters: Preparation guidance does not create a Medicaid rule.

Next step: Use the official toolkit and verify the state and plan requirements.

Policy updateIllinois

The notice announces selected code removals effective December 21 and reiterates selected additions effective January 1, including DME and orthotics. The linked change log did not establish the new code-level changes in this check. No blanket speech-therapy or AAC authorization change was verified. Government support materials distinguish MA PPO and nondelegated HMO; delegated models require separate verification.

Why it matters: Selected Medicare Advantage authorization lists are changing; speech/AAC applicability is not established.

Next step: Match the member product and exact code to the payer list and delegated review route.

BCBSIL updates physical medicine and rehabilitation policy
Posted Oct 2 · Announced Oct 20 · Effective Dec 31, 2026
Policy updateIllinois

Why it matters: The pending policy may affect applicable billing and documentation; SLP-specific implications are not confirmed.

Next step: Check the pending policy against your product and contract before its listed effective date.

BCBSIL posted pending policy RP040 on October 2 and announced it October 20; the listed effective date is December 31, 2026. The policy addresses physical medicine and rehabilitation billing and documentation. Its implications for SLP services, 2027 codes and contracted payment require plan-specific verification; a page 3 comparison now confirms a substantive maintenance-therapy text revision. Current CPCP040 (effective November 7, 2025) excludes maintenance and requires expected improvement; pending RP040 instead permits skilled maintenance to preserve function or prevent or slow deterioration. This is not a confirmed SLP benefit expansion. The documents focus on physical-medicine/PT/OT procedures and do not establish which SLP services or member products are affected.

Old/new policy text compared October 30, 2026. Listed future effective date is not evidence of implementation today.

Policy updateIllinois

Why it matters: This is a portal reminder, not proof of a new therapy authorization rule.

Next step: Confirm the member, payer and delegated review route before submitting.

BCBSIL describes how Illinois Medicaid providers can check whether authorization or notification is required and submit requests handled by BCBSIL through Medicaid Authorizations in Availity. Draft requests last only until the end of the day.

What to verify: Select the correct payer and member, and confirm the applicable service and any delegated review route. This notice is a reminder, not evidence of a new therapy authorization requirement. Authorization is not a guarantee of payment.

Oct 8, 2026ASHA Swallowing Services SurveyASHA update Oct 8 · Review underway
Policy updateNational

Why it matters: The review informs possible future valuation recommendations; it does not change payment by itself.

Next step: If personally invited, check the survey instructions and deadline. Do not forward individual survey links.

ASHA reports surveys concerning fiber endoscopic evaluation and swallowing services, and audiometric tests, as part of the AMA RUC process. Randomly selected members may receive an individual survey; links should not be forwarded.

What this means: This review gathers evidence for possible future Medicare payment recommendations. It does not itself establish a payment change, and the notice does not confirm that a survey remains open.

Why it matters: ASHA identifies potential communication-access concerns about a proposal, not a final change.

Next step: Check the federal docket for later action; the recorded October 6 comment deadline has passed.

HHS proposed replacing the Head Start performance standards. ASHA identifies potential effects on hearing screening and multilingual communication supports, including AAC. These are concerns about a proposal, not confirmed changes in effect.

BCBSIL says reviews began October 2 for Illinois Medicaid claims, including claims paid January 1, 2026 onward within the applicable recoverable lookback period. The notice describes rule-based recovery edits; it does not identify a speech-specific edit.

Policy updateWashington / Alaska

Status: Public payer guidance; individual claims/payment not confirmed.

Premera announces replacement of 92507 with ten time-based codes and directs clinicians to document direct care, avoid counting the same time twice, and use the combined category when treating speech and language together. The notice does not establish contracted rates, authorization crosswalks or benefit-limit conversion. Its statement that AMA has not released the codes should not be treated as current code-set status.

Why it matters: Coding guidance alone does not settle payment or authorization readiness.

Next step: Request product-specific rates, authorization handling and benefit counting before transition.

Sep 25, 2026Early Intervention rate changes: check the affected servicesHFS notice Sep 25 · Proposed effective Oct 1 · Federal approval unconfirmed
Policy updateIllinois

Why it matters: The proposals and published schedule have different scopes; do not assume every discipline received an increase.

Next step: Match your service and code to the schedule and verify the federal approval status.

HFS proposed increases for several EI services, including 22% for audiology and 2% for aural rehabilitation, effective on or after October 1, 2026. The proposal is subject to federal approval; speech therapy is not listed among the proposed increases. Approval status was not confirmed in this check.

IDEC separately published its FY 2027 provider rate schedule effective for dates of service on or after October 1. Check the applicable service and code rather than assuming all disciplines received an increase.

Why it matters: This is Medicaid habilitative coverage guidance, not an Early Intervention rate announcement.

Next step: Check the official notice and the child’s plan-specific coverage and authorization process.

HFS confirms medically necessary habilitative services for children under 21, including stuttering, effective for dates of service on or after July 1, 2026. The notice applies to Medicaid fee-for-service and HealthChoice Illinois MCOs. This is Medicaid coverage, not an Early Intervention rate notice.

Selected therapy authorization requirements: verify the plan
ASHA update Sep 4 · UHC list effective Oct 1 · Verify plan exceptions
Policy updateNational

Why it matters: The recorded documents conflict; product and service exceptions matter.

Next step: Verify the exact member, plan and service in the payer portal before relying on an authorization exemption.

UHC’s reduction list, effective October 1, lists 92507 and 92508 for Medicare Advantage and D-SNP, and 92526 for Medicare Advantage. ASHA reports these selected codes no longer require prior authorization under the listed products.

Unresolved: UHC’s separate October 1 requirements document still lists 92507 and 92508 for outpatient therapy. The documents conflict; verify the exact member, plan, setting and service in UHC’s portal. Requirements for the 2027 replacement codes are not confirmed here. Do not assume this applies to Medicaid managed care.

The highest practice-expense component is paid at 100%; secondary practice-expense components at 50%. This is not a 50% cut to each entire secondary service. Applies to Commercial, IPP Host and FEP; excludes IPP Home. Not a BCBSIL policy.

Why it matters: Same-group PT, OT and SLP services may interact under this methodology.

Carelon’s current rehabilitation guideline history records a July 21, 2026 clarification to a speech-therapy exclusion while retaining the original effective date. A prior-text comparison has not established exactly which wording changed or its claims impact. The current guideline distinguishes skilled maintenance-program design/instruction from ongoing maintenance treatment. Anthem adoption depends on market, product and delegated program.

Why it matters: A review-date revision can matter even when the original effective date stays unchanged.

Next step: Compare the archived version and verify the member’s adopted guideline before applying the exclusion.

Watch 277CA rejection codes A7 and 255. This applies an existing edit earlier; it is not a new clinical coverage exclusion or a payment denial. Payer policy and reimbursement news

UHC October prior authorization reductions

Announced September 1 · effective October 1, 2026 · current. UHC’s Medicare Advantage reduction list includes 92507, 92508 and 92526; its separate D-SNP list includes 92507 and 92508. The header requires checking plan-specific exceptions through the portal. The commercial list did not contain 92507 or 92526 in the retrieved text.

Practice impact: Review the exact member product, service date and code before requesting approval. This change may reduce administration for listed services but does not guarantee benefits/payment or establish approval rules for the new 2027 base/add-on family.

Premera speech treatment transition

Published October 1 · January 1, 2027 transition · Washington provider notice. Premera describes ten timed replacement codes and direct-care documentation. It directs providers to avoid counting the same time twice and use combined speech/language coding when appropriate. Rates and authorization carryover are unspecified. The notice’s AMA-release statement is outdated relative to ASHA’s October 9 report; use current ASHA code guidance. This comparison does not establish Illinois plan implementation.

Premera notice

Illinois pediatric stuttering coverage

HFS notice September 23 · effective July 1, 2026 · current. HFS confirms medically necessary habilitative coverage for early-acquired speech-language disorders, including stuttering, for children under 21 in Medicaid FFS and HealthChoice Illinois MCOs. The notice says J-200 will be updated later. Coverage does not itself establish acceptance of a new 2027 code or its rate.

Practice impact: Compare applicable denials with the dated notice and retain the specific product, service and denial reason for review. HFS notice.

BCBSIL pending rehabilitation policy

RP040 posted October 2 · approved September 21 · effective December 31, 2026. The retrieved document concerns physical medicine/rehabilitation billing and did not establish new SLP code adoption. Review applicable requirements; do not apply PT/OT timing rules to the new SLP family without code-specific authority. A pending effective date is not a comment opportunity.

Major practice impacts and comment periods

Medicare 2027 payment rule

CMS-1848-P displayed July 14 · published July 16 · proposed · comments closed September 14, 2026. CMS’s proposal concerns 2027 Medicare Part B payment and coverage. ASHA’s current transition guidance distinguishes AMA’s new code family from CMS’s proposed valuation; final Medicare values depend on the final rule. Do not treat proposed rates as final contracted or payable amounts.

Practice impact: Keep reimbursement models in scenarios and reconcile final values, code edits and payer rules before January claims. CMS rule record and downloads · ASHA transition guidance.

Head Start standards proposal

Federal proposal August 7 · ASHA analysis October 2 · proposed · comments closed October 6. ASHA identifies potential effects on hearing/developmental screening processes, multilingual AAC and assessment, staffing ratios and disability access. These are concerns about a proposal, not implemented changes. ASHA reports submitting comments.

Practice impact: Monitor final agency action and local implementation. Preserve individualized, culturally and linguistically appropriate communication supports and distinguish language differences from disorders.

ASHA analysis · Federal proposal and comment deadline

Pennsylvania AAC advocacy result

ASHA report October 1 · Pennsylvania comparison. ASHA and PSHA advocacy led Jefferson Health Plans to reverse restrictions requiring a six-month unsuccessful speech therapy trial and lack of benefit from alternative communication before considering a medically necessary SGD. This is a useful advocacy example, not an Illinois coverage change.
